CONSENT TO THE PROCESSING OF PERSONAL DATA FOR THE PURPOSE OF RECEIVING SKANZENÍK

I consent to the collection, storage, and processing of the personal data contained in this form by the data controller, Boii, z.s., for the purpose specified below. I grant this consent for all data contained in this form for a period of 10 years from the date of granting consent.

I have reviewed all completed sections of this form; all information is accurate and truthful and is provided voluntarily. All data provided in this form will be collected and processed exclusively for the purpose of sending commercial communications to the data subject via electronic means in accordance with Act No. 480/2004 Coll. This will continue until the data subject directly and effectively notifies the controller that he or she no longer wishes to receive commercial communications from the controller, but for no longer than 10 years.

Aggregated data from this form may be used by the controller for statistical purposes, specifically for the controller’s internal use. The controller declares that it will collect personal data to the extent necessary to fulfill the specified purpose and will process it only in accordance with the purpose for which it was collected.
